We are looking for a professional with a clear commitment to patient care, focused on comprehensive patient attention, the promotion of healthy lifestyles, the management of chronic conditions, and work in home and community settings. ### Job description Reporting to the Management of the Basic Health Area (ABS), the professional will assume the following responsibilities: - **Comprehensive and Continuous Medical Care:** In-person, telephone, and home care consultations for scheduled assistance and spontaneous/urgent demand. - **Health Promotion and Prevention:** Individual and community health education activities (schools, institutes, and local institutions), vaccination campaigns, and priority care for vulnerable groups and risk groups. - **Chronic Patient Management:** Early diagnosis, control, and continuous monitoring of chronic pathologies (heart disease, diabetes, hypertension, respiratory pathologies, and non-complex mental health), optimizing treatment review to avoid polypharmacy. - **Clinical and Document Management:** Use of corporate healthcare software (E-CAP / HES), issuance of electronic prescriptions, management of Temporary Disability (sick leave/return to work), medical certificates, and interpretation of complementary tests (ECG, spirometry, blood tests, X-rays, etc.). - **Multidisciplinary Coordination:** Close work with nursing, social work, hospital network specialists, mental health (ASSIR/PSP), and socio-sanitary centers. - **Quality and Teaching:** Participation in clinical meetings, quality commissions, review of care indicators (EQA / IQPF), and continuous training/teaching for residents or students. ### Essential requirements - Degree in Medicine (homologated if applicable). - Specialist title via MIR (or official homologation) in **Family and Community Medicine**. - Active registration with an Official College of Physicians in Spain. - Ability to organize consultations and proficiency in medical software. ### Valuable requirements - Previous experience in Primary Care Teams (EAP) or health centers. - Postgraduate training or master's degree in Primary Care or Pre-hospital Care. - Knowledge of the Catalonia health model (SISCAT network) and the E-CAP / HES computer system. - Knowledge of Catalan or willingness to learn it. ### What we offer - **Job stability:** Permanent full-time employment contract in a prestigious institution. - **Stable schedules:** Structured schedule typical of Primary Care that favors personal and professional balance. - **Competitive conditions:** Remuneration according to the collective agreement of the concerted health sector (SISCAT) + care supplements. - **Professional development:** Continuous learning environment, possibility of participating in community research projects, and excellent working climate in the province of Girona. ## **The company** **Actual Talent** We are looking for a **General Practitioner / Family Medicine Specialist** to join a reference Basic Health Area (ABS) in the province of Girona.
